Before you come see us on your first visit:
- Please verify that the Women's Healthcare Associates clinician you wish to see is contracted with your insurance plan.
- If needed, obtain a referral from your primary care physician.
- Make payment arrangements if you do not have insurance coverage.
- Write down any questions or concerns you wish to discuss with us. (Click here to download a form that may help you prepare in advance.)
Because we value your opinion, you will receive a brief online survey within one week following your new patient appointment. We thank you in advance for taking the time to complete the survey and helping us to consistently exceed your expectations.
On the day of your appointment, please bring:
- Your insurance card
- Your completed Medical History form (pdf*). For teens and parents, please complete the Adolescent Medical History and review the Teen Screen Parent Letter.
- A list of all medications you are currently taking
- A list of questions or concerns you may have
- A deposit for services rendered, unless your insurance plan is an HMO or PPO, in which case we request your co-payment only. We accept checks and credit cards (Visa and Mastercard).
Medical Record Release Form:
This helps us gather your records from your previous healthcare provider so that you don't have to repeat a lot of your historical information.
HIPAA (Health Insurance Portability and Accountability Act):

About your appointment:
Except for urgent medical situations, we see patients by appointment only. We will work hard to honor your appointment time and intend to keep you informed of any delay. Should your clinician be called away for a delivery or medical emergency, you will be offered the option of rescheduling your appointment, waiting to see your clinician, or seeing another clinician who is available in the office. Please be assured that all of our clinicians offer the same excellent level of service.
If you are not able to keep your appointment, please notify your Women's Healthcare Associates clinician. We appreciate 24 hours notice.
